Plan human review and escalation
Define who checks the work, which failures stop the process, and where judgment cannot be delegated.
Human review needs a job description. Specify the reviewer, evidence they inspect, time budget, rejection reasons, escalation path, and authority to stop the workflow.

Assign a role with the subject knowledge and time needed to catch meaningful errors.

Create rejection reasons
Use a small set of observable failure categories instead of a vague quality score.

Set hard stops
Block automation when the input, output, confidence, or impact crosses a defined boundary.

Review the review
Sample approved work and revise the checklist when new failure patterns appear.
